Neuropathie bei Diabetes: immer diabetische Polyneuropathie?

Zusammenfassung

Wenn ein Mensch mit Diabetes an Polyneuropathie leidet, handelt es sich zwar nicht immer um eine diabetische Polyneuropathie, aber sehr häufig. In Anbetracht der hohen Prävalenz von Diabetes mellitus und der ebenso hohen Prävalenz von Polyneuropathien insbesondere bei älteren Menschen ist ein zufälliges Zusammentreffen der beiden Erkrankungen allerdings ebenfalls wahrscheinlich. Die Unterscheidung zwischen einer diabetischen und einer nichtdiabetischen Polyneuropathie kann schwierig sein, und insbesondere über die behandelbaren, immun vermittelten Neuropathien bei Menschen mit Diabetes wird kontrovers diskutiert. Im vorliegenden Beitrag werden klinische Strategien beschrieben, die in der Differenzialdiagnose zwischen diabetischer und nichtdiabetischer Polyneuropathie bei Diabetes hilfreich sind.
