Zusammenfassung

Das posteriore reversible Enzephalopathiesyndrom (PRES) ist ein klinisch-radiologisches Syndrom, welches sich mit epileptischen Anfällen, Kopfschmerzen, Sehstörungen, Verwirrtheit oder fokal-neurologischen Defiziten manifestiert. In der Bildgebung stellen sich häufig Veränderungen der posterioren Hirnareale, insbesondere des Okzipital- und Parietallappens dar. PRES wird im Zusammenhang mit einem breiten Spektrum an möglichen Ursachen und Erkrankungen beschrieben, sehr häufig jedoch im Rahmen von Hypertonie, Nierenerkrankungen, Präeklampsie/Eklampsie oder nach Transplantation. Die Pathophysiologie des PRES ist noch nicht ausreichend geklärt, möglicherweise spielt die unterschiedlich bedingte Endothelschädigung eine zentrale Rolle. Die Prognose des PRES ist bei frühzeitiger intensivmedizinischer Behandlung und Beseitigung des Auslösers in der Regel gut, eine Verzögerung der Diagnose und Therapie kann jedoch zu Komplikationen mit permanenten neurologischen Defiziten führen. In der vorliegenden Übersicht werden das klinische und radiologische Erscheinungsbild, die Ursachen und Hypothesen zur Pathophysiologie sowie die Grundprinzipien der Therapie des PRES behandelt.
