Postoperative Behandlung von Fingergelenkprothesen

Zusammenfassung

Hintergrund

Der künstliche Gelenkersatz ist eine Behandlungsoption für ein verschlissenes Fingergrund- und Mittelgelenk. Künstliche Fingergelenke erfordern eine gezielte Nachbehandlung. Für ein gutes Behandlungsergebnis sind eine konsequente Mitarbeit des Patienten und die enge Zusammenarbeit zwischen Operateur und Therapeut wichtig. Es gibt jedoch keine allgemeingültigen Richtlinien für die postoperative Behandlung von Fingergelenkprothesen.

Methodik

Es werden die verschiedenen Teilbereiche der postoperativen Behandlung von Fingergrund- und Fingermittelgelenkprothesen (Ödem‑/Schwellungsbehandlung, Narbenbehandlung, Handtherapie und Schienenbau) beleuchtet und Anhaltspunkte für die Nachbehandlung von Patienten gegeben. Zudem werden Behandlungskonzepte und Ergebnisse von Fingergrund- und Fingermittelgelenkprothesen aktueller Publikationen beschrieben und diskutiert.

Ergebnisse

Trotz adäquater postoperativer Behandlung scheint sich das Bewegungsausmaß nach Implantation einer Fingergelenkprothese im Vergleich zum Ausgangsbefund nur gering zu verbessern. Hingegen ist eine deutliche Schmerzreduktion und Verbesserung der Punktwerte von Ergebnisevaluationsfragebögen im Vergleich zu präoperativen Daten zu verzeichnen.

Schlussfolgerungen

Für ein gutes Ergebnis einer Fingergelenkprothese ist die postoperative Behandlung essentiell. Die Nachbehandlung beinhaltet Ödem‑/Schwellungsbehandlung, Narbenbehandlung, Handtherapie und Schienenbau. Die postoperative Behandlung folgt Prinzipien und sollte individuell an den Zugangsweg zum Gelenk, den verwendeten Prothesentyp, die Knochenqualität und Stabilität des Weichteilmantels des betroffenen Fingers, die Compliance des Patienten und an die Verfügbarkeit eines qualifizierten Therapeuten angepasst werden.
