Präoperative Risikoevaluation aus anästhesiologischer Sicht

Zusammenfassung

Medizinischer und technischer Fortschritt sowie der demographische Wandel führen zu einer zunehmenden Komplexität bei der perioperativen Versorgung unserer Patienten. Dementsprechend kommt einer optimalen präoperativen Vorbereitung, insbesondere einer adäquaten Risikoevaluation, eine zunehmende Bedeutung zu. Eine kürzlich veröffentlichte gemeinsame Empfehlung der jeweiligen deutschen Fachgesellschaften der Anästhesiologie, der Inneren Medizin und der Chirurgie hat zum Ziel, mittels transparenter und verbindlicher Absprachen mögliche Unsicherheiten insbesondere bei den technischen Voruntersuchungen im Rahmen der präoperativen Risikoevaluation auszuräumen und ein gemeinsames Vorgehen zu ermöglichen. Damit wird ein routinemäßiges „Screening“ zugunsten einer gezielten individuellen Diagnostik zunehmend verlassen. Hierdurch soll der präoperative Ablauf unter wissenschaftlichen, organisatorischen und wirtschaftlichen Gesichtspunkten verbessert werden. Der folgende Beitrag hat zum Ziel, auf der Grundlage dieser sehr wertvollen und hilfreichen Empfehlungen weitere wichtige Aspekte bei der präoperativen Risikoevaluation aus anästhesiologischer Sicht darzustellen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
