Psychokardiologische Rehabilitation – interdisziplinäre Behandlung als Pilotprojekt

Zusammenfassung

Psychische Komorbidität kann den Verlauf einer Herzerkrankung entscheidend beeinflussen und erhöht die Mortalität signifikant. Umgekehrt können Herzerkrankungen und invasive Behandlungsmaßnahmen, wie z. B. bedrohliche Herzrhythmusstörungen oder eine Serie von ICD-Schocks, eine Angsterkrankung oder eine posttraumatische Belastungsstörung (PTBS) auslösen. Für die kardiologische Routinebehandlung und v. a. auch für die Rehabilitation sind diese Zusammenhänge von besonderer Bedeutung, da sie die psychische Komorbidität Aktivität und Teilhabe oft stärker einschränken als die organische Herzkrankheit. Diese Wechselwirkungen erfordern integrierte Behandlungskonzepte, die in unserem nach Fachdisziplinen organisierten Versorgungssystem bisher nur selten angeboten werden. Dieser Beitrag beschreibt nach einer Übersicht über ambulante und stationäre psychokardiologische Behandlungsmodelle ein solches integriertes Behandlungsmodell – die psychokardiologische Rehabilitation. Seit Ende 2016 gibt es im Rehazentrum Seehof, Teltow eine Behandlungseinheit, in der im Rahmen eines Pilotprojektes 24 Patienten interdisziplinär psychosomatisch und kardiologisch behandelt werden. Am Ende steht eine gemeinsame sozialmedizinische Beurteilung. Dargestellt werden das Behandlungskonzept sowie bisherige Erfahrungen.
