Mehr als eine Alternative? – Wann braucht man die Radiuskopfprothese?

Zusammenfassung

Frakturen des Radiuskopfes entstehen typischerweise durch eine axiale Krafteinwirkung. Die Behandlungsstrategie von Trümmerfrakturen orientiert sich an der Stabilität des Ellenbogengelenks und verlangt ein individuelles Abwägen zwischen Rekonstruktion, Resektion und prothetischem Ersatz. Bei Vorliegen eines stabilen ulnaren Seitenbandes ist bei nicht rekonstruierbarer Radiuskopftrümmerfraktur die Radiuskopfresektion die Therapie der Wahl. Besteht eine nicht rekonstruierbare Valgusinstabilität, ist die Indikation zur Implantation einer Radiuskopfprothese gegeben. Beim jungen Patienten sollte der Versuch einer Rekonstruktion großzügig unternommen werden. Die Implantation einer Radiuskopfprothese ist in jeder Situation, in der eine kopferhaltende Therapie sinnvoll erscheint, kontraindiziert. Vorbestehende lokale oder systemische Infektionen schließen eine Implantation aus. An Komplikationen sind mit einem fortschreitenden Knorpelabrieb am Capitulum, Bewegungseinschränkungen im Ellenbogen sowie heterotopen Ossifikationen zu rechnen. Zudem wird über Lysesäume im Schaftbereich berichtet, deren Auswirkung auf den klinischen Verlauf noch nicht abschließend geklärt ist.
