Patienten in Notfallambulanzen in Deutschland

Zusammenfassung

Hintergrund

Schwindel ist eines der häufigsten Leitsymptome in der Medizin. Bei akutem Schwindel ist eine interdisziplinäre Diagnostik erforderlich. Akuter Schwindel ist ein Notfall. Neue diagnostische Methoden (Videokopfimpulstest) und Algorithmen wie HINTS, die Kombination von Kopfimpulstest („head impulse test“), Nystagmus und Achsenabweichung der Augen („test of skew deviation“), setzen sich in der Praxis immer weiter durch.

Methode

In einer Umfrage zum Management und der Beteiligung des HNO-Fachgebiets haben die Autoren im Frühjahr 2017 insgesamt 168 deutsche HNO-Kliniken online befragt.

Ergebnisse

Insgesamt haben 100 der 168 angeschriebenen Kliniken an der Umfrage teilgenommen. Erstkonsultierende Fachgebiete bei akutem Schwindel sind die HNO-Heilkunde (48 %), Neurologie (32 %) und innere Medizin (17 %). Die meisten Patienten wurden stationär aufgenommen. Patienten mit akutem Schwindel verbleiben am häufigsten in der HNO-Klinik (62 %) und Neurologie (16 %), seltener in der inneren Medizin. Für die Notfalldiagnostik wird der Videokopfimpulstest in der HNO-Heilkunde häufig eingesetzt (72 %).

Schlussfolgerungen

Bei akuten Schwindelsyndromen kommt dem HNO-Arzt eine besondere Rolle und Verantwortung in der interdisziplinären Abklärung zu, da sich am häufigsten otologische Störungen als Ursache finden. Eine HNO-ärztliche Konsiliaruntersuchung ist immer zu empfehlen. Der HNO-Arzt verfügt über moderne diagnostische Mittel wie den Videokopfimpulstest, um im Notfall periphere Vestibulopathien zu objektivieren oder zentrale Ursachen abzugrenzen. Damit leistet er einen Beitrag zur Verbesserung der medizinischen Versorgung.
