Zusammenfassung

Hintergrund: Das Zytostatikum 5-Fluorouracil erfährt sowohl in der kurativen adjuvanten, als auch in der palliativen Therapie primär inkurabler Malignome zunehmende Anwendung. In den letzten Jahren häufen sich Berichte über eine kardiotoxische Wirkung des 5-Fluorouracil. Methode und Patienten: Es werden zwei Fälle kardiotoxischer Nebenwirkungen bei einer Kombinationschemotherapie (5-Fluorouracil und Carboplatin) mit simultaner Strahlentherapie bei primär inkurablen Kopf-Hals-Tumoren dargestellt. Ergebnisse: Kardiale Nebenwirkungen manifestieren sich bei etwa 1,1 - 4,5 % aller mit 5-Fluorouracil behandelten Patienten. Mögliche Pathomechanismen und therapeutische Interventionen werden dargestellt. Schlussfolgerung: Klinisch-symptomatische Manifestationen kardiotoxischer Nebenwirkungen von 5-Fluorouracil sind selten; bei kardialen Risikopatienten sollte die Infusion unter adäquatem Monitoring erfolgen. Eine strenge Indikationsstellung, sei es zur kurativen oder palliativen Therapie, ist obligat.

Cardiotoxicity of 5-Fluorouracil - An Important Side Effect in Palliative Cancer Treatment?

Background: In the past cardiotoxicity has been increasingly reported as 5-fluorouracil is widely used as well in curative adjuvant as in palliative chemotherapy of malignant neoplasms. Methods: We report on two cases of 5-fluorouracil-associated cardiotoxicity on palliative treatment of head and neck cancer and evaluate the importance of intensive drug monitoring. Results: 5-fluorouracil-associated cardiotoxicity occurs with an incidence of 1.1 - 4.5 %. Ischaemic cardiopathy due to vasospasms is believed to be the most common manifestation. Conclusions: Since most patients with head and neck cancer have risk factors for cardiovascular disease, 5-fluorouracil-associated cardiotoxicity should not be underestimated and patients should be monitored carefully.
