Diffuse Endothelialisation eines operativen Zyklodialyse-Spaltes. Licht- und elektronenmikroskopische Studie

Zusammenfassung

20 Jahre nach operativer Zyklodialyse mußte bei einem 58jährigen Patienten mit chronischer Uveitis anterior das rechte Auge wegen eines schmerzhaften Sekundärglaukoms entfernt werden. Licht- und elektronenmikroskopisch zeigte sich eine komplette diffuse Endothelialisation der Zyklodialyse-Tasche mit Ausbildung einer dicken „descemetähnlichen” Basalmembran. Der Endothelverband ist „gefen-stert”. Bei dieser Membran handelt es sich um einen sog. multilaminaren Typ der descemetähnlichen Basalmembran. Die klinische und histologische Differential-Diagnose derartiger descementähnlicher Membranen werden diskutiert. Eine Endothelialisation eines Zyklodialyse-Spalts kann somit Ursache für eine erfolglose Zyklodialyse sein.

Summary

Twenty years after surgical cyclodialysis it became necessary to remove the right eye of a 58-year-old patient with chronic unspecific anterior uveitis who had developed a painful secondary glaucoma. Light and electron microscopic investigation revealed that a complete, diffuse endothelialization with a Descemet-like basement membrane had lined the anterior chamber and the cyclodialysis cleft. The endothelial cellstructure was fenestrated and the basement membrane was a “multi-layered Descemet-like membrane”. Clinical and histological differential diagnoses of such membrane formations are discussed. Endothelialization of the cyclodialysis cleft can be the cause of an unsuccessful operation.
