Prognosefaktoren und Prognose des Endometriumkarzinoms

Zusammenfassung

Das Endometriumkarzinom kann histopathologisch und nach endokrinologischen Parametern in 2 Typen unterschieden werden. Das klarzellige und das seröse Endometriumkarzinom des Typs II haben eine deutlich schlechtere Prognose als die Vertreter des Typ-I-Karzinoms (östrogen-/progesteronrezeptorpositiv) und werden wie ein Ovarialkarzinom behandelt. Weitere Prognosefaktoren, wie die Tumorgröße unter Einbezug des Invasionsverhaltens, die Lymphknoten- und die (Fern)metastasierung, und weniger spezifische Faktoren wie Alter, Parität, metabolische Störungen und genetische Aberrationen beeinflussen das Rezidivierungsverhalten und das Überleben der betroffenen Patientin. Die Entdeckung neuer molekularbiologischer und molekulargenetischer Prognosefaktoren eröffnet möglicherweise neue Diagnose- und Therapiewege und trägt zu einer Verbesserung der Prognose betroffener Patientinnen bei.
