Prophylaxe von Neuralrohrdefekten mit Folsäure

Zusammenfassung

Trotz Rückgangs der Inzidenz von Neuralrohrdefekten wird das mögliche Potenzial der Folsäureprophylaxe in Europa nicht ausgenutzt. Zahlreiche Untersuchungen der letzten Jahrzehnte haben die Möglichkeiten der Reduktion des Risikos für Neuralrohrdefekte und anderer angeborener Fehlbildungen aufgezeigt. Durch die perikonzeptionelle Zufuhr von 400 µg Folsäure pro Tag bei allen Frauen mit Kinderwunsch bzw. von 4 mg pro Tag bei Frauen, die schon ein Kind mit einem Neuralrohrdefekt geboren haben, ist eine Reduktion der Inzidenz um 70–80% möglich. Die Probleme mit der Optimierung der Zufuhr von natürlichen Folaten und die fehlende Umsetzung der Empfehlungen zur Folsäuresupplementierung lassen die generelle Anreicherung eines Grundnahrungsmittels (Mehl, Salz) als Ausweg erscheinen. Durch die engen Zusammenhänge des Folat- und Homocysteinstoffwechsels sind zudem positive Effekte auf kardiovaskuläre Erkrankungen der Bevölkerung zu erwarten.
