5. Primärstaging des Prostatakarzinoms

Zusammenfassung

Es werden die Prostatakarzinommanifestationen in den verschiedenen zonalen Drüsenanteilen sowie dessen Multifokalität, Differenzialdiagnosen, Pitfalls und morphologische Charakteristika dargestellt. Die Karzinomstadien werden anhand der TNM-Klassifikation mit Bildbeispielen unter Einbeziehung der PI-RADS-V2-Kriterien referiert. Es erfolgt eine Bewertung der diagnostischen Genauigkeit der MRT für die Beurteilung eines extraprostatischen Wachstums. Zur Optimierung des Primärstagings stehen als nuklearmedizinische Verfahren die PET/CT mit 18F-Fluorid, 18F-Cholin sowie 18F-PSMA zur Verfügung. Die Fluorid-PET/CT kann die Skelettszintigrafie ersetzen und in ihrer Aussagekraft übertreffen. Mittels Cholin-PET/CT kann das Lymphknoten- und Metastasen-Staging des Prostatakarzinoms mit höherer Genauigkeit als mit CT und MRT erfolgen. Studien zu neu entwickelten PSMA-Tracern konnten eine deutliche Überlegenheit gegenüber der Cholin-PET/CT nachweisen. Die PSMA-PET/CT hat das Potenzial, sich zur Standarduntersuchungsmethode sowohl zur Primär- als auch zur Rezidivdiagnostik zu entwickeln.

Krebsassoziierte venöse Thromboembolien (CAT) haben in den vergangenen Jahren stetig zugenommen. Was hat der Anstieg mit modernen Antitumortherapien zu tun?
