Der Roboter in der Urologie – eine Analyse aktueller und zukünftiger Gerätegenerationen

Zusammenfassung

Die in den letzten 15 Jahren existierende Monopolsituation bezüglich der Roboter für die laparoskopische Chirurgie wird 2019 enden, da dann wesentlichen Patente von Intuitive Surgical auslaufen. Somit kann in den kommenden Jahren eine interessante Konkurrenzsituation erwartet werden. Basierend auf persönlichen Erfahrungen mit der roboterassistierten Chirurgie seit 2001, führten wir eine Literaturrecherche durch, ergänzt durch die Studie relevanter Patente für die robotische Chirurgie. Außerdem besuchten wir die Webseiten der Hersteller aktueller und zukünftiger Operationsroboter mit relevanten Einsatzmöglichkeiten in der Urologie. Diese müssen erst belegen, ob sie dem hohen Qualitätsstandard der Da Vinci®-Serie (SI, X, XI) entsprechen. Hierbei können Vergleichskriterien wie Art der Konsole (offen/geschlossen), Anordnung der Roboterarme (auf einem oder mehreren Wägen), Art des dreidimensionalen (3D-)Videosystems (durch Spiegeltechnik, Okular oder mittels Brille) und die Qualität der Endeffektoren (5 vs. 7 Freiheitsgrade) hilfreich sein. Inzwischen stehen auch Robotersysteme für die Endourologie zur Verfügung: Das Avicenna Roboflex® (Elmed, Ankara, Türkei) für die flexible Ureteroskopie und das AquaBeam®-System (Procept BioRobotics, Redwood Shores, CA) für die roboterassistierte Aquaablation der Prostata. Während Roboflex® ähnlich dem Da Vinci®-System die Ergonomie des Eingriffs erleichtert, ersetzt die robotergestützte Aquablation erstmals den Operateur, abgesehen von der möglichen Blutstillung.
