Bewegungsanalyse und muskuloskeletale Simulation in der Pseudarthrosentherapie – Erfahrungen und erste klinische Ergebnisse

Zusammenfassung

Hintergrund

Die mechanischen Rahmenbedingungen des Pseudarthrosen- und Osteosynthesenkonstrukts sind wesentliche Determinanten der Heilung nach Revisionseingriffen. Ziel der Arbeit ist es, einen Analyse- und Simulationsablauf zur In-vivo-Bestimmung der mechanischen Bedingungen im Frakturspalt darzustellen.

Methodik

Auf individueller Behandlungsbasis wurde nach Revisionseingriffen zur Pseudarthrosentherapie zunächst eine Bewegungsanalyse mit einem akzelerometriebasierten Messsystem durchgeführt. Im Anschluss wurde die Analyse in eine muskuloskeletale Simulation überführt, um die mechanischen Rahmenbedingungen der Versorgungssituation sowie der angrenzenden Gelenke darzustellen.

Ergebnisse

Insgesamt liegen klinische Behandlungsverläufe von 13 Patienten vor. Mit dem vorgestellten Mess- und Simulationsablauf lassen sich die mechanischen Verhältnisse in vivo darstellen. Bei allen Patienten kam es zur radiologischen Konsolidierung der Fraktur.

Diskussion

Der vorgestellte Messablauf erlaubt eine klinisch nutzbare Darstellung der mechanischen Rahmenbedingungen der Fraktur- und Pseudarthrosenheilung. Weitere Studien können nun den Nutzen dieser neuen Messtechnik auch in der postoperativen Nachbehandlungsempfehlung und biomechanisch adaptierten Versorgungsstrategie, basierend auf den gezeigten Ergebnissen, untersuchen.
