Zusammenfassung

Pseudarthrosen entwickeln sich in Abhängigkeit von vorhandenen Risikofaktoren in etwa 10 % als Folge einer Fraktur und sind gehäuft im Bereich der langen Röhrenknochen lokalisiert. Die Ausbildung einer Pseudarthrose geht für den Patienten regelhaft mit einer prolongierten Schmerzsymptomatik und reduzierten Funktionalität der betroffenen Extremität einher. Die resultierenden deutlich erhöhten sozioökonomischen Kosten sind einerseits auf die aufwendige Behandlung, andererseits besonders auf die indirekten Kosten durch Arbeitsausfall und Krankengeld zurückzuführen. Insgesamt erfordern diese komplexen Verletzungsfolgen ein differenziertes und individualisiertes therapeutisches Vorgehen. Die Analyse der Pseudarthrose auf Basis des „Diamant-Konzepts“ ist der Schlüssel für die weitere Therapieplanung. Unter bestimmten Voraussetzungen ist die Anwendung physikalischer Maßnahmen wie die extrakorporale Stoßwellentherapie möglich. In den meisten Fällen ist allerdings eine operative Revision mit Optimierung der biomechanischen Stabilität und/oder der lokalen Biologie unumgänglich.
