Zusammenfassung

Hintergrund

Die Psoriasisarthritis (PsA) gilt als Prototyp der peripheren Spondyloarthritiden (SpA) und geht als „skin-bone disease“ (SkiBo) mit typischen Hautveränderungen und Arthritiden an den Gelenken, insbesondere der Hände und Füße, einher.

Ziel der Arbeit

Systematische Aufarbeitung der bildgebenden manuellen Befundmuster bei der PsA mit ihren typischen enthesitischen Arthritiden im Kontext mit den klinischen Befunden.

Material und Methoden

Übersichtsarbeit auf dem Boden der aktuellen Literatur zum Thema aus radiologischer und rheumatologischer Sicht.

Ergebnisse

Entzündliche Manifestationen an Händen und Füßen stellen bei der PsA die häufigsten betroffenen Regionen am Körper dar. Bildmorphologisch zeigen sich Variabilitäten zwischen Früh- und Spätstadien. Das projektionsradiographische Kennzeichen der PsA ist das simultane Nebeneinander von osteodestruierend-rarefizierenden mit typischen osteoproliferativ-produktiven Veränderungen, die Bestandteil der „Classification of Psoriatic Arthritis“-Kriterien (CASPAR) sind. Mit Hilfe der Magnetresonanztomographie (MRT) können die bildmorphologischen Spondyloarthritis-Entzündungsmuster einer Enthesitis, Daktylitis und Osteitis an den Händen bereits in Frühstadien nachgewiesen werden.

Diskussion

Die Basiskenntnisse der bildmorphologischen Veränderungen dieser komplexen autoimmunologischen „skin-bone disease“ werden vermittelt, um die notwendige Sicherheit in der systematischen diagnostischen Evaluation der für die PsA typischen manuellen Arthritismuster sowohl bei der Erstdiagnose als auch im Erkrankungsverlauf sowie in der Einordnung von Früh- und Spätstadien zu erlangen, welche den Rheumatologen als Weichenstellungen für Step-up-Therapien dienen.
