Zusammenfassung

Herzchirurgische Eingriffe bedeuten neben der Hoffnung auf Genesung und Verbesserung der Lebensqualität emotionalen Stress für die Patienten. Das Belastungserleben beeinflusst den postoperativen Genesungsprozess. Außer der möglichen physischen Gefährdung stellen Kontrollverlusterleben, Komplikations-, Todesängste und Schmerzen die Hauptstressoren dar. Psychosoziale Risikofaktoren (niedriger sozioökonomischer Status, Arbeitsstress, soziale Isolation, Depression, Angst, posttraumatische Belastungsstörungen) wirken sich auf die koronare Herzkrankheit und das Outcome nach Bypassoperation negativ aus. Viele Patienten nach implantiertem Kardioverter/Defibrillator, linksventrikulärem Unterstützungssystem oder nach Herztransplantation weisen ebenfalls psychische Erkrankungen auf. Aufgrund der Relevanz psychosozialer Faktoren sollte ein systematisches Screening erfolgen. Die psychotherapeutische Mitversorgung sollte herzchirurgischen Patienten mit psychischen Komorbiditäten dringend angeboten werden.
