Psychologisches Prophylaxetraining zur Bewältigung postoperativer Schmerzen

Langfristige Effekte

Zusammenfassung

Einleitung

In der vorliegenden Studie wurde der Effekt eines multidimensionalen psychologischen Prophylaxetrainings zur kognitiv-emotionalen Schmerzbewältigung auf den Genesungsverlauf in den ersten 12 Monaten nach einer Operation untersucht. Das Training umfasste die folgenden drei Bausteine: Edukation über Schmerz, Analgesie und psychische Aspekte der Schmerzbewältigung; Schmerzbewältigungstraining und körperzentrierte Entspannungsübungen.

Material und Methoden

Insgesamt 48 männliche Patienten wurden am Tag vor der operativen Korrektur einer Brustwanddeformität, am Entlassungstag sowie 3, 6 und 12 Monate nach der Operation untersucht. Erfasst wurden die Schmerzintensität und Beeinträchtigung durch Schmerzen sowie Schmerzangst, -katastrophisieren und -hypervigilanz. Die Hälfte der Patienten (n = 24) nahm am Tag vor und 1–3 Tage nach der Operation an einem je 1-stündigen Training zur kognitiv-emotionalen Schmerzbewältigung teil.

Ergebnisse

Der Anteil an Patienten mit klinisch relevanten Verbesserungen war in der Prophylaxegruppe signifikant höher als in der Kontrollgruppe. Dies bezog sich auf die Akutschmerzintensität (etwa eine Woche nach der Operation), die Beeinträchtigung durch Schmerzen nach 3 Monaten und die Schmerzangst nach 12 Monaten.

Schlussfolgerung

Ein Wiederansteigen der Schmerzangst nach 12 Monaten zeigte sich nur in der Kontrollgruppe und kann möglicherweise auf die anstehende Operation zur Entfernung des brustwandstabilisierenden Metallimplantats zurückgeführt werden. Das Prophylaxetraining ist daher als Protektivfaktor für den langfristigen Umgang mit Folgen der Operation und späteren Schmerzerlebnissen anzusehen.
