Psychotische Störungen durch Alkohol – eine eigene diagnostische Entität?

Zusammenfassung

Bei der psychotischen Störung durch Alkohol handelt es sich um ein relativ seltenes Krankheitsbild mit schizophrenieartiger Symptomatik, das durch die Leitsymptome (akustische) Halluzinationen, Angst und Verfolgungswahn bei fehlenden Bewusstseins- und Orientierungsstörungen oder Entzugserscheinungen charakterisiert ist. Die diagnostische Entität dieser Störung und die Prognose wurde lange Zeit kontrovers diskutiert. Aktuelle Untersuchungen belegen zum einen eine hohe Rückfallgefahr hinsichtlich stationärer Wiederaufnahmen, aber ein insbesondere im Vergleich zu anderen substanzinduzierten Psychosen geringes Risiko für den Übergang in eine schizophrene Psychose (5 % gegenüber 40–50 % bei anderen Drogen). Pathophysiologische, diagnostische, Verlaufs- und therapeutische Aspekte der alkoholinduzierten psychotischen Störungen werden diskutiert.
