A 23-year-old male was evaluated in the ED with severe abdominal pain and bloody diarrhea. Computed tomography (CT) of the abdomen and pelvis showed pancolitis with the most severe inflammation seen in the distal colon; subsequent flexible sigmoidoscopy revealed severe colitis to the extent of the exam. Histological evaluation confirmed chronic inflammatory colitis with moderate activity. He was diagnosed with ulcerative colitis (UC) (Montreal classification E3). Oral prednisone 40 mg daily was initially prescribed followed by vedolizumab 300 mg every 8 weeks. Despite these therapies, he ultimately required hospitalization due to symptom persistence. …
