We read with interest the paper by Prangenberg et al. “Fox sign in a case of terminal stage pancreatic cancer and suggestions for diagnosis” [1]. We commend the authors for their novel approach using a pancreatic enzyme lipase immunoassay in order to confirm the relationship between necrotizing pancreatitis and cutaneous ecchymosis within the tissue at the right flank and below the right inguinal ligament [1]. Herein, we further expand upon the clinical findings and clarify the associated conditions and pathophysiologic process of this sign. …
