Despite advances in therapies, the treatment paradigm for limited-stage small cell lung cancer (LS-SCLC) has remained relatively stagnant over the previous two decades.1,2 With this in mind, treatment approaches have typically included systemic therapy and radiation therapy, including prophylactic cranial irradiation, as the mainstays of therapy. Despite slow progress in treatment modalities for this patient population, long-term oncologic outcomes have improved, with approximately one-third of patients surviving 5 years.1 While historically there were limited data to support the role of surgical resection in patients with LS-SCLC, more contemporary data suggest that a carefully selected cohort of patients with LS-SCLC may benefit from surgery as part of a multimodality approach.3,4 Given the limited number of patients who may be candidates for surgical resection, there are few prognostic tools to guide discussions regarding expectations in this patient population. Furthermore, there is a paucity of information regarding dynamic changes in prognosis in lung cancer that may evolve as patients survive longer from initial diagnosis.5,6 …
