Regadenoson, a selective A2A receptor agonist with weak affinity for the A1 receptor, is commonly used as a pharmacologic stress agent for myocardial perfusion imaging. Activity at the A1 receptor remains important as these receptors are present in the sinoatrial (SA) and atrioventricular (AV) nodes and may account for the few reported cases of atrioventricular node block or asystole after regadenoson infusion.1 However, to date, no cases of regadenoson-induced bradycardia and asystole followed by hyperacute T-waves have been reported. …
