Eine akute Pyelonephritis im Säuglings- und Kindesalter kann Nierenparenchymdefekte hervorrufen, insbesondere wenn die Diagnose nicht zeitnah nach dem Auftreten der Symptome gestellt wird und die antibiotische Therapie verzögert beginnt. Erst kürzlich wurde in der schwedischen Refluxstudie erneut das hohe Risiko von segmentalen Nierenparenchymschäden bei Säuglingen und Kleinkindern mit akuter Pyelonephritis gezeigt [9]. Die Empfänglichkeit für Rezidive ist in den ersten Monaten nach der ersten fieberhaften Harnwegsinfektion am größten und korreliert auch mit der Anzahl der vorausgegangenen Infektionen [2]. Etwa ein Drittel der Kinder erleiden ein Rezidiv, und deshalb wird bei einem vesikoureteralen Reflux eine Reinfektionsprophylaxe empfohlen [4, 5]. …
