Reliability of Polysomnography Study at the Time of Discharge from Hospital Among COPD-AE Patients: A Longitudinal Feasibility Study

Abstract
Background
Conventionally, screening for suspected OSA in patients presenting with COPD-acute exacerbation (COPD-AE) is done after 4–6 weeks. This study was done to find out whether polysomnography can be done at the time of discharge from hospital among patients with COPD-AE and to find out the prevalence of sleep disorders in COPD-AE.
Methodology
Patients presenting with COPD-AE after stabilization underwent full-night diagnostic level-I PSG followed by full-night titration study (if AHI ≥ 15). Subjects with OSA (AHI ≥ 5) were asked to undergo repeat PSG, followed by a full-night titration study (if AHI ≥ 15) after 4–6 weeks of discharge to compare PSG characteristics.
Results
Fifty-seven subjects {median Age: 60 [IQR 55–67] years, median BMI: 23 [IQR 19.3–26.7] kg/m2; median FEV1: 0.84 Liter (IQR 0.66–1.07)} were enrolled for sleep study at the time of discharge. Insomnia and Restless Leg Syndrome (RLS) was seen in 87% and 49%, respectively. In the initial sleep study, 94.7% of subjects with COPD had OSA and 3/4th had AHI ≥ 15. Subjects with AHI ≥ 5 at baseline were followed up with repeat PSG after 4–6 weeks; 93% of subjects (n = 30) had moderate–severe OSA. Follow-up PSG variables did not vary significantly from baseline PSG. There was no significant change in AHI and PAP pressure from baseline to follow-up PSG in most patients.
Conclusion
This study with a limited sample size showed PSG variables done at the time of COPD-AE do not vary significantly compared to when repeated at a relatively stable period; however, it needs to be studied in a larger population. OSA, RLS, and Insomnia are highly prevalent in COPD-AE.
