Reosteosynthese vs. Prothese nach fehlgeschlagener Humeruskopfosteosynthese

Zusammenfassung

Komplikationen nach osteosynthetischer Versorgung einer proximalen Humerusfraktur sind häufig und können oft komplexe Revisionseingriffe erforderlich machen. Vor Planung einer Revision ist zunächst eine umfangreiche klinische und bildgebende Diagnostik erforderlich, um die Symptomatik des Patienten und die morphologischen Veränderungen des Knochens und der Weichteile vollständig zu erfassen. Ist eine Revision erforderlich, muss die Entscheidung zur Revisionsosteosynthese (ggf. in Kombination mit einer Korrekturosteotomie) oder zum Gelenkersatz abhängig von verschiedenen Faktoren wie Patientenalter, Zeitraum seit Versorgung, Zustand der Tuberkula und der anheftenden Rotatorenmanschette getroffen werden. Klare Algorithmen existieren nicht. Die Behandlung erfolgt individuell auf den jeweiligen Patienten abgestimmt. Während junge Patienten ohne signifikante Arthrose von einer Korrekturosteotomie profitieren können, wird beim älteren Patienten mit symptomatischen Komplikationen und Frakturfolgen zunehmend häufiger die inverse Prothese eingesetzt.
