Zusammenfassung

Mit der zunehmenden Verbreitung extrakorporaler Lungenersatzverfahren (ECLA) haben in den letzten Jahren auch unterschiedliche Decarboxylierungssysteme Eingang in die intensivmedizinische Praxis gefunden. Diese Verfahren stellen aufgrund der hohen CO2-Transportkapazität des Bluts geringere Anforderungen an den extrakorporalen Blutfluss und ermöglichen eine effektive Decarboxylierung bei gleichzeitig deutlich reduzierter Invasivität und Komplexität. Neben Systemen, die sich technisch aus der Lungenersatztherapie ableiten und in erster Linie zur Behandlung der schweren hyperkapnischen respiratorischen Insuffizienz eingesetzt werden, stehen in zunehmendem Umfang Verfahren zur Verfügung, die auf klassische Nierenersatzplattformen zurückgehen („respiratory dialysis“). Auch in diesem Niedrigflussbereich ist eine effektive partielle extrakorporale CO2-Elimination möglich, die sowohl zur Kontrolle einer respiratorischen Acidose beitragen als auch protektive und ultraprotektive Beatmungsstrategien beim akuten Lungenversagen (ARDS) ermöglichen oder unterstützen kann. Obwohl derartige Therapieansätze mit einer Reduktion der beatmungsassoziierten Lungenschädigung einhergehen können, fehlen bisher Daten, die einen positiven Effekt auf harte klinische Endpunkte, wie Mortalität oder Beatmungsdauer, belegen. Daher muss der Einsatz von extrakorporalen Decarboxylierungssystemen derzeit als experimentell gelten und sollte Zentren mit entsprechender Erfahrung und Expertise vorbehalten bleiben.
