Retroperitoneales Hämangioperizytom – ein seltener Fall mit mehrfachen Rezidiven – selbst 20 Jahre nach Erstdiagnose

Zusammenfassung

Ein gesunder 41-jähriger Mann stellte sich in unserer Klinik mit neu aufgetretenen Schmerzen im Unterbauch und intermittierenden Flankenschmerzen vor. Es zeigte sich ein Tumor des kleinen Beckens unklarer Dignität. Die Histologie erbrachte den Nachweis eines niedrig malignen Hämangioperizytoms. Es wird unter dem Überbegriff solitärer fibröser Tumor (SFT) subsumiert. In unserem seltenen Fall zeigten sich mehrmalige retroperitoneale Rezidive und eine zunehmende Proliferationsaktivität des Tumors sowohl nach 14 als auch nach 20 Jahren. Das Malignitätspotenzial ist sehr variabel. Für die Rezidiv- und Metastasierungsneigung sowie für das Gesamtüberleben steht eine Risikostratifizierung zur Verfügung. Ein individualisiertes Therapiemanagement und engmaschige sowie langjährige Nachsorgen dieser seltenen Tumorentität sind essenziell.
