Rezidivierende Schlaganfälle und systemische Ischämien unklarer Ätiologie

Eine 56-jährige Patientin wurde aufgrund neu aufgetretener diskreter Wortfindungsstörungen in unserer neurologischen Notfallambulanz vorgestellt. Die Patientin befand sich aktuell in der neurologischen Rehabilitation aufgrund einer Hypästhesie und Feinmotorikstörung der linken Hand, die auf zerebrale Manifestationen einer bekannten Kollagenose zurückgeführt wurden. Ein Jahr zuvor war extern bei Arthralgien, Serositis und erhöhten Antikörpertitern die Diagnose einer Mischkollagenose (differentialdiagnostisch Sharp-Syndrom, systemischer Lupus erythematodes oder systemische Sklerose) gestellt und seither immunsuppressiv therapiert worden. Vier Wochen vor der aktuellen Vorstellung waren erstmals Ischämien im rechten Mediaterritorium diagnostiziert worden. Wir nahmen die Patientin aufgrund der nun neuen Wortfindungsstörungen auf unsere Stroke-Unit auf und führten die vorbestehende Medikation mit ASS 100 mg/Tag und die aufgrund der Kollagenose extern begonnene Immunsuppression (Methotrexat 7,5 mg/Woche, Hydroxychloroquin 200 mg/Tag, Prednisolon aktuell 70 mg/Tag) zunächst unverändert fort. …
