Periphere Nervenbeteiligung bei rheumatischen Erkrankungen

Zusammenfassung

Periphere Neuropathien sind eine wahrscheinlich unterdiagnostizierte Komplikation rheumatischer Erkrankungen. Sie können klinisch blande verlaufen, jedoch ebenfalls zu ausgeprägten Einschränkungen der Lebensqualität führen. Eine präzise Diagnose und ätiologische Zuordnung sind für die Therapie und Prognose peripherer Neuropathien von großer Bedeutung. Anamnese und körperliche Untersuchung stellen das Fundament für eine weiterführende Diagnostik dar. Während elektrophysiologische Untersuchungen beim Verdacht auf eine periphere Neuropathie obligat sind, ist nur in Einzelfällen eine Nerven- oder Nerven-Muskel-Biopsie indiziert. Therapiekonzepte werden weiterhin durch fehlende Evidenz erschwert. Sie entsprechen häufig erprobten immunsuppressiven Therapien der Grunderkrankungen wie Glukokortikoide, Cyclophosphamid, Mycophenolat-Mofetil und intravenösen Immunglobulinen und lehnen sich zudem an die symptomatische Schmerztherapie anderer Neuropathien an. Hier stehen als Erstlinientherapie Gabapentin, Pregabalin, Duloxetin, Venlafaxin und trizyklische Antidepressiva zur Verfügung.
