Rhinosinusitis in der Pädiatrie und bei Lungenerkrankungen

Wissensstand und die neue AWMF-Leitlinie

Zusammenfassung

Die akute Rhinosinusitis (ARS) ist häufig. Typisch ist das Auftreten nach einer viralen Infektion der oberen Atemwege und die Ausheilung binnen 10 Tagen – mit und ohne Therapie. Oft besteht ein Zusammenhang der ARS oder auch der chronischen Rhinosinusitis (CRS) mit Erkrankungen der unteren Atemwege. Rezidivierende Pneumonien kommen häufiger bei CRS mit abtropfenden Rachensekreten („postnasal drip“) als bei gesunden Kindern vor. Ursache eines anhaltenden Hustens kann ein „postnasal drip syndrome“ oder Asthma bronchiale sein, ggf. in Kombination. Auch ist an eine protrahierte bakterielle Bronchitis, psychogenen Husten und an eine gastroösophageale Refluxerkrankung als Ursache zu denken. Darüber hinaus spielen die atopischen Erkrankungen eine Rolle. In der vorliegenden Arbeit wird der aktuelle Wissensstand zusammengefasst. Außerdem wird die gerade veröffentlichte AWMF-Leitlinie „Rhinosinusitis“ der Deutschen Gesellschaft für Allgemeinmedizin und Familienmedizin und der Deutschen Gesellschaft für Hals-Nasen-Ohren-Heilkunde, Kopf- und Hals-Chirurgie im Abgleich mit Empfehlungen der European Society of Pediatric Otorhinolaryngology (EPOS) erörtert.
