Riboflavin-UVA-Crosslinking bei progredientem Keratokonus

Zusammenfassung

Bei Patienten mit Keratokonus kommt es im Krankheitsverlauf zu einer fortschreitenden Verformung der Hornhaut. Die damit eingehergehende Myopisierung und der zunehmende, v. a. auch irreguläre Astigmatismus werden in der Regel zunächst mit einer Brillenkorrektur und später mit formstabilen Spezialkontaktlinsen symptomatisch behandelt. Die Diagnosestellung erfolgt in der Regel mithilfe der kornealen Tomographie anhand spezifischer Befundkonstellationen. Nach Erstdiagnose sollten regelmäßige Kontrollen mittels Tomographie durchgeführt werden. Sollte sich eine klinisch signifikante Progression objektiv feststellen und reproduzierbar nachweisen lassen, sollte den Betroffenen ein Riboflavin-UVA-Crosslinking angeboten werden. Ziel dieser Behandlung ist es, das Fortschreiten zu stoppen und so Spätfolgen zu vermeiden. Das Prinzip der Behandlung besteht in der Wechselwirkung des Photosensibilisators Riboflavin und UVA-Licht. Die Effektivität dieser Behandlung konnte in verschiedenen randomisierten kontrollierten Studien nachgewiesen werden.
