We read an interesting manuscript by Dichtl [
1], who reported volumetric and functional changes of the right insular cortex (Ic) based on anatomical and resting-state functional magnetic resonance (fMRI) imaging in Takotsubo syndrome (TTS). Although the relationships of the right anterior Ic atrophy and the left amygdala atrophy with sympatho-vagal autonomic instability were suggested to underlie in TTS, several points should be taken into account to understand the pathophysiology of TTS in relation to central autonomic network (CAN) presented by Dichtl [
