Zusammenfassung

Hintergrund

Die steigende Anzahl an Implantationen von Kniegelenktotalendoprothesen korreliert mit der Zunahme von Revisionsoperationen und periprothetischen Frakturen.

Ziel

In dieser Arbeit sollen neben der Klassifikation der periprothetischen Fraktur am Kniegelenk Strategien zur Vermeidung von Komplikationen nach ORIF („open reduction internal fixation“) periprothetischer Frakturen aufgezeigt werden.

Therapiestrategien

Die osteosynthetische Versorgung kommt nur bei einem Teil der Fälle in Frage. Obwohl spezielle periprothetische, winkelstabile Osteosyntheseplatten zur Verfügung stehen, verbleibt die Versorgung periprothetischer Frakturen am Kniegelenk eine Herausforderung mit einer hohen Komplikationsrate. Ist die Prothese locker, sollte keine ORIF erfolgen, sondern ein Prothesenwechsel.

Risikofaktoren

Zu den Risikofaktoren für das Versagen einer Osteosynthese zählen das Prothesenmodell und die damit verbundene Reduktion verbliebenen Knochens und die meist osteoporotische Knochenstruktur.
