Fibromuscular dysplasia (FMD) is a noninflammatory, nonatherosclerotic arterial disease that can cause stenosis, occlusion, dissection, and aneurysm development in systemic arterial beds [1]. A potentially unique pathology of subarachnoid hemorrhage (SAH) with a higher prevalence of intracranial aneurysms has been reported in patients with FMD. Herein, we present an atypical clinical manifestation of SAH caused by rupture of a very small de novo posterior inferior cerebellar artery (PICA) aneurysm in a patient with FMD. …
