To the Editor: In premature newborns, a variety of etiologies result in early hypotension [
1]. Transient adrenal insufficiency (TAI) is one etiology which is difficult to diagnose in this group [
2]. ACTH test is superior to random cortisol levels to diagnose TAI [
3]. In a former study, an increase in serum cortisol below 12 % from baseline during ACTH study had 93 % specificity for detecting hypotension that was treated with inotropes, and thus suspected to be a clinical sign of adrenal insufficiency [
