Präsakrale Tumoren

Zusammenfassung

Präsakrale Tumoren sind selten und werden aufgrund ihrer versteckten Lage im kleinen Becken spät und damit oft in einem fortgeschrittenen Stadium entdeckt. Die zu Beginn meist unspezifischen Symptome werden häufig fehlgedeutet und die eingesetzte Schnittbilddiagnostik erfasst in vielen Fällen nicht den gesamten Präsakralraum. Deshalb sollte jede Schnittbilddiagnostik des Abdomens den Präsakralraum einbeziehen. Mit der Magnetresonanztomographie gelingt bei entsprechender Expertise eine exzellente histologische Einordnung der Tumoren. Eine bioptische Untersuchung zur histologischen Einordnung ist nur in ausgewählten Fällen erforderlich. Trotz der histologischen Vielfalt präsakraler Tumoren ist die chirurgische R0-Resektion der Goldstandard in der Therapie. Die frühe Diagnose vermeidet ausgedehnte Eingriffe und erhöht die R0-Resektions-Rate. Die multidisziplinäre Behandlung sollte in Zentren erfolgen, die über Erfahrung in der fortgeschrittenen Tumorchirurgie am kleinen Becken verfügen.
