Schädel-Hirn-Trauma unter antithrombotischer Medikation

Hämostaseologische Therapie bei intrakraniellen Blutungen

Zusammenfassung

Störungen der Hämostase stellen bei traumatischen intrakraniellen Blutungen entscheidende Risikofaktoren für ein schlechtes Outcome sowie eine erhöhte Mortalität dar. Im Rahmen eines Polytraumas mit großen Blutverlusten kann es zu einer Hyperfibrinolyse und damit zu einer ausgedehnten Koagulopathie kommen. Erleiden Patienten unter der Behandlung mit antithrombotischen Medikamenten intrakranielle Blutungen, sind sie aufgrund der erhöhten Blutungsneigung besonders gefährdet, permanente neurologische Schäden zu erleiden. Dieser Artikel soll dem Leser die Grundlagen der hämostaseologischen Therapie traumatischer intrakranieller Blutungen unter antithrombotischer Medikation erläutern. Neben der Skizzierung pathophysiologischer Grundlagen wird auf die Auswirkungen von Gerinnungsstörungen bei diesem Patientenkollektiv eingegangen. Des Weiteren werden die Anwendung spezieller Gerinnungstests sowie die Gabe hämostatischer Substanzen erläutert.
