Zusammenfassung

Die obere Atemwegstimulation spielt in der Therapie der obstruktiven Schlafapnoe (OSA) eine immer wichtigere Bedeutung. Ziel der Stimulation ist der N. hypoglossus (N. XII), der als rein motorischer Nerv die Zungenmuskulatur innerviert. Durch die selektive Stimulation einzelner Nervenfasern kann der obere Atemweg mithilfe einer Protrusion der Zunge geöffnet werden. Der N. XII weist eine Vielzahl von anatomischen Varianten auf, die v. a. bei der chirurgischen Implantation dieser Schrittmachersysteme entscheidend sind. Das intraoperative Neuromonitoring ist hier sehr hilfreich. Für die genaue Platzierung der Stimulationselektrode bei der selektiven oberen Atemwegstimulation sind nun die Kenntnisse der Anatomie des N. XII, das intraoperative Neuromonitoring und die genaue Beurteilung von Muskelkontraktionen und Zungenbewegungen von entscheidender Bedeutung.

COMISA - Die komorbide Insomnie bei Schlafapnoe

Insomnie und obstruktive Schlafapnoe (OSA) treten häufiger zusammen auf, als man es basierend auf der jeweiligen Prävalenz in der Bevölkerung vermuten würde [1, 2, 3]. In der Fachliteratur nennt sich diese Komorbidität COMISA – comorbid insomnia and sleep apnea.

Dieser CME-Kurs macht Sie mit den Definitionen von Schicht- und Nachtarbeit sowie den potenziellen gesundheitlichen Auswirkungen einer Schichtarbeitstörung vertraut. Er gibt weiterhin einen Überblick über mögliche Auswirkungen verschiedener Schichtmodelle auf die Schlafdauer und zeigt verschiedene Therapieoptionen auf.

Schlafstörungen sind sehr häufig bei älteren Menschen. Die physiologischen Veränderungen des Schlafs müssen von den Schlafstörungen im engeren Sinne abgegrenzt werden. Dieser CME-Kurs gibt Ihnen einen Überblick, wie Sie die Besonderheiten älterer Menschen mithilfe des geriatrischen Assessments erkennen und Schlafprobleme in den Kontext der Multimorbidität einordnen.
