Zusammenfassung

Schlafbezogene Atmungsstörungen (SBAS) stellen eine bedeutsame Komorbidität bei Patienten mit kardialen Arrhythmien dar. Eine Assoziation zwischen SBAS und sowohl supraventrikulären als auch ventrikulären Rhythmusereignissen konnte in diversen Studien dokumentiert werden. Im Kollektiv herzinsuffizienter Patienten wurde zudem ein kürzeres ereignisfreies Überleben bis zum Auftreten der ersten lebensbedrohlichen Arrhythmie bei zeitgleich bestehender, unbehandelter SBAS nachgewiesen. Mögliche Kausalitäten scheinen insbesondere bei der obstruktiven Schlafapnoe in wiederkehrenden Hypoxämien, erhöhten mechanischen Beanspruchungen (Wandspannung) mit erhöhtem Energieverbrauch und Weckreaktionen („arousals“) mit nachfolgender Aktivierung des sympathischen Nervensystems zu liegen. Bei der Cheyne-Stokes Atmung werden die pathophysiologischen Zusammenhänge derzeit noch nicht vollständig verstanden, aber auch hier scheinen Hypoxämien und Sympathikusaktivierungen eine Rolle zu spielen. Erste kleinere Interventionsstudien zeigen ein positives Ergebnis hinsichtlich der Suppression von Rhythmusereignissen durch eine adäquate Therapie der SBAS, hier stehen jedoch randomisierte und kontrollierte Studien noch aus.
