Schlafbezogene Atmungsstörungen und Vorhofflimmern

Zusammenfassung

Schlafbezogene Atmungsstörungen stellen eine der häufigsten schlafmedizinischen Diagnosen dar. Insbesondere die obstruktive Schlafapnoe (OSA) gilt als unabhängiger kardiovaskulärer Risikofaktor. Untersuchungen konnten eine signifikante Assoziation zwischen OSA und Vorhofflimmern als häufigster Herzrhythmusstörung zeigen. Der Nachweis einer direkten Kausalität ist noch ausstehend.
Eine unbehandelte OSA ist ein Risikofaktor für das Versagen einer Kardioversion. Auch das Rezidivrisiko nach erfolgreicher Rhythmuskontrolle ist durch eine nicht therapierte OSA erhöht und kann durch die Einleitung einer spezifischen Überdrucktherapie (CPAP) reduziert werden. Es fehlen noch entsprechende randomisierte, kontrollierte, klinische Studien mit definierten Endpunkten.
Neben einer schlafmedizinischen Anamnese ist bei Verdacht auf eine schlafbezogene Atmungsstörung eine leitliniengerechte Diagnostik und Therapieeinleitung essenziell. Entsprechende Handlungsempfehlungen werden formuliert.
