Facettenreiches Problem Postamputationsschmerz

Ein 76-jähriger Mann, dem bei schwerer arterieller peripherer Verschlusskrankheit (pAVK) vor fast drei Jahren der Unterschenkel proximal amputiert wurde (Abb. 1), klagt seit einem Jahr über Stumpfschmerzen, die das Tragen einer Prothese unmöglich machten. Aufgrund der bekannten pAVK wurde einige Monate nach Beginn der Schmerzen eine digitale Substraktionsangiografie (DSA) der Beckengefäße über die linke A. femoralis communis durchgeführt, die damals rechts eine hochgradige Abgangsstenose und eine distalen Verschluss der A. femoralis superficialis rechts ergeben hatte. Nach antegrader perkutaner transluminaler Angioplastie (PTA) besserten sich die Schmerzen jedoch nur geringfügig. Eine daraufhin veranlasste neurologische Untersuchung zeigte damals am Unterschenkelstumpf eine erhöhte Druckempfindlichkeit ohne Hautulzerationen oder Druckläsionen. Bei einer ein Jahr vorher aufgetretenen Herpes-Zoster-Radikulitis S1 links fanden sich keine Hinweise für herpetiforme Hauteffloreszenzen oder eine post-herpetische Neuralgie als Schmerzursache. Bereits vor der Amputation hatten über Jahre brennende Missempfindungen an beiden Füßen bestanden, wobei die Sensibilität bis auf eine Pallhypästhesie mit 5/8 am Großzeh links unauffällig gewesen war. Unter der Annahme eines neuropathischen Schmerzsyndroms erfolgte damals eine Behandlung mit Gabapentin, die ebenfalls nur zu einer geringen Besserung geführte hatte. …
