Schwere intrahepatische Cholestase bei einem 66-jährigen Patienten mit medikamentös behandeltem Vorhofflimmern

Zusammenfassung

Ein 66-jähriger Patient stellte sich mit schmerzlosem Ikterus bei bekannter ischämischer Kardiomyopathie mit intermittierendem Vorhofflimmern, rezidivierenden Kammertachykardien und wiederholtem Kammerflimmern vor. Seit 2 Jahren erfolgte deshalb eine orale Antikoagulation mit Phenprocoumon sowie eine Amiodarontherapie (200 mg/Tag). Laborchemisch fanden sich eine deutliche Erhöhung der Cholestaseparameter und Transaminasen. Serologisch konnten infektiöse, autoimmunologische bzw. metabolische Lebererkrankungen ausgeschlossen werden. Sonographisch sowie in der ERCP ergab sich kein Anhalt für eine mechanische Cholestase, einen Tumor im Bereich des Pankreas oder eine kardiale Leberstauung. Histologisch wurde eine schwere intrahepatische Cholestase gesichert, vereinbar mit einer medikamentös-toxischen Leberschädigung. Unter Pausierung von Phenprocoumon kam es zu einem weiteren Anstieg der Leberwerte, die erst nach Absetzen von Amiodaron deutlich rückläufig waren. Zur Prävention maligner Herzrhythmusstörungen wurde der Patient mit einem atrioventrikulären Defibrillatorsystem versorgt. Die intrahepatische Cholestase stellt eine seltene Nebenwirkung von Amiodaron dar, die auch nach längerfristiger problemloser Einnahme auftreten kann.
