Selection of the right implant pocket often is difficult and has been the ongoing topic of discussion in the literature. Currently, a multitude of implant choices are available that can be combined with available pockets to fit an individual need. As often said by Dr. Tom Biggs, “No one key fits all the locks.” However, longevity, and aesthetic outcome are influenced by breast envelope stability. An augmented breast envelope in a submuscular plane or subfascial pocket is stable and better supported. Muscle and pectoral fascia in these planes provide support and prolong aesthetic results. …
