Zusammenfassung

Das ROSE-Konzept, ein Akronym für „resuscitation“, „optimization“ (Optimierung), „stabilization“ (Stabilisierung) und „evacuation“ (Volumenentzug), beschreibt eine an die Pathophysiologie des septischen Schocks angelehnte Flüssigkeitstherapie in der Intensivmedizin. Die ersten beiden Phasen sind insbesondere durch eine aggressive, klinisch und hämodynamisch gesteuerte Flüssigkeitstherapie charakterisiert. In der Phase der Stabilisierung ist die Erholung vom Schock und von der Mikrozirkulationsstörung zu erkennen, was es ermöglicht, in der vierten und letzten Phase wieder Volumen zu entziehen. Ziel ist es, dass das interstitielle Ödem mit seinen gewebsschädigenden Effekten zurückgeht und der Zustand der Euvolämie wiederhergestellt wird.
