A 13-year-old girl with chronic active Epstein–Barr virus infection received unrelated cord blood transplantation (CBT) after a preparative regimen comprising rabbit antithymocyte globulin (ATG, 2.5 mg/kg/day for 2 days), fludarabine (30 mg/m2/day for 6 days), melphalan (70 mg/m2/day for 2 days), and total body irradiation (2 Gy/day for 2 days). Tacrolimus and methylprednisolone were used for graft-versus-host disease prophylaxis. Intravenous acyclovir (ACV) was infused at 10 mg/kg twice a day for prophylaxis against herpes simplex virus (HSV) infection. …
