Sexuelle Differenzierungsstörungen und proximale Hypospadie

Zusammenfassung

Hintergrund

Kinder mit einem differenten chromosomalen, gonadalen oder genitalen Geschlecht stellen sowohl diagnostisch als auch therapeutisch eine besondere Herausforderung dar. Eine Abgrenzung dieser Kinder mit DSD („disorders of sex development“) zu proximalen Hypospadien mit normalem phänotypischem männlichem Genitale ist sowohl bezüglich der geschlechtsspezifischen Identität als auch im Hinblick auf die chirurgische Therapie wesentlich.

Ziel

Dieser Artikel gibt einen Überblick über den aktuellen Wissensstand sowie diagnostische wie therapeutische Empfehlungen auf Basis der nationalen wie internationalen pädiatrisch-endokrinologischen sowie kinderurologischen Diskussion zur Abklärung und Behandlung der sexuellen Differenzierungsstörungen sowie proximalen Hypospadien.

Material und Methoden

Die verschiedenen Formen des DSD vor dem Hintergrund der embryologischen wie pathophysiologischen Zusammenhänge werden in Reflexion der internationalen Literatur in einen Kontext zu den klinisch relevanten Erscheinungsformen der Intersexualität gesetzt. Die Diagnostik und Therapie von Patienten mit proximalen Hypospadien werden dabei intensiv diskutiert.

Ergebnisse

Aufgrund der diagnostischen wie therapeutischen Komplexität der somatischen geschlechtlichen Entwicklung wie geschlechtsspezifischen Sozialisation von Kindern mit DSD ist bei proximalen Hypospadien mit fraglicher gonadaler Übereinstimmung ein interdisziplinäres Expertenteam, bestehend aus pädiatrischen Endokrinologen, Molekularbiologen, klinischen Psychologen und Kinderurologen, unter vorrangiger Einbeziehung der Eltern für die Entscheidungsfindung einer spezifischen Therapie obligat.
