Spezielle Aspekte der Pränataldiagnostik im 1. Trimenon bei Schwangerschaften nach assistierter Reproduktion

Zusammenfassung

Nach Maßnahmen der assistierten Reproduktion finden sich ein durchschnittlich höheres Lebensalter der Schwangeren sowie eine höhere Inzidenz an Mehrlingsschwangerschaften. Beide Faktoren erhöhen das Risiko für fetale Aneuploidien. Gegenwärtige nichtinvasive Screeninguntersuchungen basieren auf Risikokalkulationen unter Einbeziehung des maternalen Alters sowie biochemischer und sonographischer Untersuchungen im 1. und 2. Trimenon. Da das biochemische Serumscreening vor allem im 2., teilweise aber auch im 1. Trimenon bei Schwangerschaften nach assistierter Reproduktion signifikant höhere falsch-positive Resultate als nach Spontankonzeption liefert, würde dies zu einer erhöhten Rate an invasiver Diagnostik führen. Deshalb sollte die sonographische Untersuchung bevorzugt werden. Eine effiziente Screeningmethode zur Identifikation von Feten mit einem hohen Aneuploidierisiko ist die fetale Nackentransparenzmessung im 1. Trimenon. Diese Untersuchung ist auch bei Zwillingen und höhergradigen Mehrlingen anwendbar.
