Die eitrige Spondylodiszitis: Symptome, Diagnostik und Behandlungsstrategien

Zusammenfassung

Hintergrund

Die eitrige Spondylodiszitis ist eine seltene Erkrankung die in den letzten Jahren immer häufiger diagnostiziert wird. Sie ist mit einer hohen Morbidität und Mortalität verbunden.

Diagnose

Durch ihre unspezifische Symptomatik wird die eitrige Spondylodiszitis häufig erst mit einiger Verzögerung diagnostiziert. Neben einem Erregernachweis ist die MRT als Goldstandard zielführend. Ergänzend kann zur operativen Planung und zu späteren Verlaufskontrollen eine Röntgenuntersuchung und/oder ein CT vorgenommen werden. Bei fehlendem Keimnachweis im Blut oder anderem Gewebe sollte die offene Biopsie der CT-gesteuerten Biopsie vorgezogen werden.

Therapie

Die Therapie kann konservativ mittels Ruhigstellung und adäquater Antibiotika- und Schmerztherapie oder chirurgisch erfolgen. Bei neurologischen Defiziten, Instabilitäten oder Deformitäten der Wirbelsäule ist die chirurgische Therapie indiziert. Die operativen Behandlungsstrategien hängen dabei von der Schwere der Erkrankung ab.

Ausblick

Die Prognose ist abhängig von einer schnellen Diagnose und einem zügigen Begin der Therapie. Eine eindeutige Evidenz im Bezug auf die Therapie (konservativ vs. chirurgisch besteht nicht.
