Zusammenfassung

Hintergrund

Heutige visuelle Untersuchungsmethoden des Kehlkopfes und der Stimmlippenbewegung erlauben vorwiegend eine zweidimensionale Darstellung der real dreidimensionalen Strukturen und Bewegungen. Die vertikale Bewegungskomponente der Stimmlippen wird meist vernachlässigt, was zu einem Informationsverlust führt. Neuere Studien zeigen, dass die vertikale Komponente in einer ähnlichen Größenordnung wie die laterale Komponente liegt und einen nicht unerheblichen Anteil am Phonationsprozess hat.

Ziel der Arbeit

In der vorliegenden Arbeit wird ein Verfahren zur zukünftigen 3D-Rekonstruktion und Visualisierung endoskopisch aufgezeichneter Stimmlippenschwingungen präsentiert.

Material und Methoden

Das Setup beinhaltet eine Hochgeschwindigkeitskamera (HGK) und ein Laserprojektionssystem (LPS) welches ein regelmäßiges Punktegitter auf die Stimmlippen projiziert. Das System ermöglicht die 3D-Rekonstruktion der Stimmlippenoberflächen und quantitative Aussage über dreidimensionale Auslenkungseigenschaften und Geschwindigkeitswerte. Die Anwendbarkeit der Methodik wird jeweils an einem humanen Kadaverkehlkopf, einem Schweinekehlkopf sowie an einem künstlichen Kehlkopfmodell mit Silikonstimmlippen demonstriert.

Ergebnisse

Der Versuchsaufbau ermöglicht die Rekonstruktion der Topographie der sichtbaren Oberfläche beider Stimmlippen zu jedem Aufnahmezeitpunkt der HGK, wodurch sich die dreidimensionale Oberflächenbewegung der Stimmlippen während der Phonation berechnen lässt. Das Setup erlaubt die quantitative Analyse der Stimmlippenphysiologie bzgl. Auslenkungen, Geschwindigkeiten und Beschleunigungseigenschaften.

Diskussion

Das nächste Ziel ist die Miniaturisierung des LPS für klinische Untersuchungen am Menschen. Hier erwarten wir uns neue Erkenntnisse über die Zusammenhänge zwischen 3D-Schwingungseigenschaften und akustischer Signalqualität bei gesunder und kranker Stimmgebung.
